Very mean receptionist!
I have no comment on the docs as I never really got to see any. But, one of the receptionists, an older lady ... what a bad attitude! Will take all her stresses out on you so beware. Not only rude and lacking basic people skills, but demanded the SS# like her life depended on it. Completely ignored my remark that insurance companies now provide alternative ID #s for the safekeeping of SS and prevention of identity theft. Insisted on it so much I had no clue what to think and ideas started crippling as to why she needed it so badly. Threatened with "bills will be sent to your house!" despite the fact that I calmly explained that that was the policy of the insurers and not my gimmick. One very angry lady!
